As a person of full legal age and having full capacity under law, I hereby grant my consent to the Foundation for the Development of the Education System hosting SALTO Eastern Europe and Caucasus Resource Centre, seated in Warsaw, Al. Jerozolimskie 142A for the use of my image (face, clothing, behaviour, voice, statements, way of expressing thoughts), which the Foundation may record in the form of photographs and audio-visual works during events organised with the participation of the Foundation and concerning the statutory activity of the Foundation.
